“My 600-Lb Life” is about to return to TLC and it’s slated to be one of the most engaging seasons yet. For Season 5, TLC followed 15 people as they tried to beat their addiction to food by getting a gastric bypass, which drastically reduces the size of someone’s stomach.

“Now, my life is a nightmare,” the first participant says in the trailer. The preview shows how difficult it is for people over 600 pounds to bathe.

One of the subjects, who has severe sores on his legs, says he’s been bedridden for more than three years.

There’s a first for Season 5. Twin sisters will get the life-saving surgery. “It’s just so hard to do anything,” one of the sisters says. “And it’s the same way for my twin.”

Even though each one of the subjects wants to get help and lose the weight, their addiction to food hinders them. “It’s the cruelest form of addiction,” one person says. The subjects are shown eating unhealthy foods like cheeseburgers and ice cream.

While their health is at risk, being “super morbidly obese” effects the whole family. In one case, a single mother has five children to take care of. At more than 600 pounds, it’s hard to take care of them but she cannot control the addiction.

But surgery is only the beginning. Once the patients get the procedure, they must follow through with the exercise and diet plan. “It doesn’t look like he lost any weight,” Dr. Younan Nowzaradan tells a family member. “You’re killing him!”

One of the twin sisters suffers a medical emergency. “What’s wrong with my sister?” Brandi yells as her sister is wheeled away on a hospital bed. “I love you, Kandi.” The participant’s heart stopped and she might have brain damage, Brandi is later informed about her sister.

Season 5 of “My 600-lb Life” will premiere Wednesday, Jan. 4 at 8 p.m. EST on TLC.
