"16 and Pregnant" star Jordan Cashmyer's cause of death has been revealed over a month after her passing.

Cashmyer died of fentanyl and cocaine intoxication, according to a statement to E! News from the Office of Chief Medical Examiner for the Maryland Department of Health. She was 26 years old when she died on Jan. 15, her family previously said.

However, the manner of her death remains undetermined, so it is unclear if her death was accidental or not.

Cashmyer's rep previously confirmed the former MTV star's death in a statement to E! News on Jan. 16.

"This is an incredibly sad day. Jordan was a sweet and caring person," the rep said. "Last we spoke she was doing well and seemed to be in a good place mentally and physically. I wish people had the opportunity to know her beyond her struggles."

Last month, the TV personality's father Dennis Cashmeyer Jr. also shared a tribute to his daughter via Facebook and launched a GoFundMe page to raise funds for Jordan's final arrangements and her 7-month-old daughter Lyla's future care.

"Last night, I received a call no parent ever wants," read the Facebook post, which also included a black-and-white photo of Jordan. "My oldest beloved daughter, Jordan, has passed away. She was only 26 years old. Our hearts are truly broken. No parent should ever have to go through losing a child, EVER."

"Please keep my family in your thoughts prayers as we navigate through this terrible tragedy," the post, which also tagged Jordan's mom Jessica Cashmyer, added. "Please understand and respect our privacy at this time. We are surrounded by the love and support of our children, granddaughter Lyla and family members."

On the GoFundMe page, Dennis wrote, "Addiction plagues many families, and our family was not immune to it."

He described his daughter as someone who was "always so full of life and full of personality," adding that "she had a way of always making those around her laugh and smile, and her smile was truly contagious."

According to Dennis, Jordan's daughter Lyla also lost her father to addiction when the child was just 2 months old.

Jordan appeared on "16 and Pregnant" in 2014 alongside her then-boyfriend Derek Taylor and their daughter Genevieve "Evie" Shae Taylor.

Their episode showed Jordan's struggles with unemployment, homelessness and being disowned by her parents, who did not approve of her relationship with Taylor.

Jordan and Taylor later split, and she signed over temporary custody rights of Evie to Taylor's mother in 2015. Two years later, Jordan was arrested for drug possession, Deadline reported.

Prior to her death, Jordan celebrated one year of sobriety in January 2021 and seemed to be dedicated to getting well, according to the outlet.
