Suzanne Hart, 41, was crushed in a freak elevator accident at 285 Madison Ave. in mid-town Manhattan this morning. She had stepped into the elevator on the ground floor, when it suddenly started to go up with its door open, trapping her inside, according to a report by The New York Post.

According to the FDNY, Rescue 1 got two other passengers out and pronounced Hart dead at the scene. The two passengers were taken to NYU Medical Center for trauma.

The building is home to advertising agency Young & Rubicam Brands, as well as other companies.

The case is under investigation, the paper reported.
